New Patient Checklist

Excellent care happens when the physicians and office staff are aligned with your family's priorities and philosophies regarding medical care. Only then can shared decision-making be based on mutual respect.  It is crucial that you have given serious consideration to the fit between our practice philosophies and your own when choosing a pediatric practice for your children.


Please read the following checklist carefully to be sure Valley Pediatrics is the appropriate medical home for your children.

  • Medical Home

    We work hard to provide comprehensive medical care and serve as your medical home. To that end, we expect that you contact our office FIRST before seeking specialty care, or heading to another provider for urgent care. We want to be involved in either providing care in our office where appropriate, or referring you to the most appropriate specialist and helping to coordinate your care.


    Whenever you do see a specialist, we ask that you request a report be sent directly to our office so we may stay informed and have the most up-to-date information in your medical record.

  • Office Hours

    We make every effort to meet the needs of our patients, offering both routine well visits and sick visit appointments. We will see any sick child the same day when necessary. We do offer walk-in appointments if necessary. However Walk-In appointments are seen in the first available sick visit appointment slot. 


    To ensure we provided you with the most efficient care we highly recommend you call ahead to schedule an appointment.

  • Scheduled Appointments

    For acute care, call the office to speak with our staff, who will listen to your concerns and schedule an appointment. If an appointment is not needed, the staff will advise you regarding your issue and send a message to the clinician. 


    For well visits, please call the office. Well visits may be scheduled up to 3 months in advance. You should receive reminders several days prior to the appointment.  

  • After Hours

    We have someone on call 24/7/365. When our office is closed overnight and you have an urgent issue that cannot wait until morning, call our main number. The message will indicate how to leave a message or contact someone.

  • Timeliness & Missed Appointments

    Our practice makes every effort to run on time with appointments, as we believe everyone’s time is equally valuable. As a courtesy, we will remind you of your upcoming appointments via phone/text message/email.


    We ask that you arrive 15 minutes before your scheduled appointment time to complete paperwork & surveys, pay co-pays, have your child weighed, measured, etc., so the patient is ready for the clinician on time. We understand sometimes things happen beyond your control that may cause you to be late. Please call as soon as you can. However, we reserve the right to ask you to reschedule if you arrive late for your appointment.


    Missed Appointments: Broken appointments represent a slot that could have been offered to another patient. If you are unable to keep your scheduled appointment, please call our office 24 hours before your appointment to reschedule. This will allow time to provide that time slot to another patient.  We reserve the right to charge for appointments that are not canceled at least 24 hours in advance or missed with no notice. After the third consecutive missed appointment you may discharged from the practice. New patients, a fee may be charged if the FIRST appointment is missed.

  • Technology

    Our practice prides itself on efficiency through use of technology. You will be encouraged to consult our website, register for and use our patient portal.  We use automated reminders for appointments that enable you to easily confirm appointments. We also send reminders when routine care/immunizations that are due. You will be asked to complete surveys prior to check ups to streamline the visit and focus on YOUR most important concerns.

  • Vaccines

    We are stong advocates for vaccination and we feel this is the best way to protect all children from preventable diseases. All patients are Valley Pediatrics must recieve all required vaccines. If you chose to not vaccinate, we will ask you to leave the practice to protect other children who are too young or have contraindications for vaccines. 


    Our Vaccine Policy follows the schedule outlined by The Centers for Disease Control and Prevention (CDC) and The American Academy of Pediatrics (AAP).

  • Antibiotics

    Antibiotics are an extremely valuable tool, but can lose effectiveness when overused. We work hard to not overuse antibiotics. We educate families on appropriate use of antibiotics, and follow evidence-based guidelines. 


    We do not routinely prescribe antibiotics over the phone as we do not believe that is good medicine. We will prescribe an antibiotic when we believe it is an appropriate treatment.

  • Insurance

    Make sure we participate with your insurance plan. 


    It is your responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays at the time of service. This is a requirement of the insurance company.


    Our billing staff will do their best to assist you with insurance questions; however, If you have questions about your coverage, it is best check with your specific insurance company. Our office does not want you to be surprised by a bill, but must always bill your health plan based on federal guidelines and the actual services provided.

  • Billing

    Insurance copays are expected to be paid at the time of service. This is an insurance company requirement. If you are unable to comply, please speak with the billing department prior to the visit to set up a payment plan.

  • Forms

    Forms are required for so many things in a child's life: day care, school, sports, camp, college and more. We will fill out forms only if your child is up to date with check ups. There may be a fee.
